Accessible Home Health Care of Houston

Accessible Home Health Care of Houston utilizes a team approach to home health care, (both medical and non-medical). Our primary goal is to enhance the "Quality of Life" of the families we serve. All participants involved in the care of the families we serve work together to achieve specific goals, (patients / family members / physicians / nurses / therapists / aides). We create a Plan of Care, then we measure our performance to the plan.
